Summary:
As of today, Cardiovascular diseases (CVD) are still the leading cause of death in Germany. Hereby, the separation in Germany between inpatient and outpatient care is one of the major obstacles to the availability of patients’ health information. The aim of CAEHR2 is to link health data across sectors throughout the entire treatment pathway - from prehabilitation through hospitalisation and rehabilitation to outpatient aftercare - with a focus on patients with cardiovascular diseases.
Within this context, the DHRG will conduct an analysis of different solutions to determine how the hub can be operated in a long-term and sustainable manner.
